  7. What they actually doDenials, appeals, underpayment follow-up and credentialing: included or extra.
  8. Reporting you're owedWhat they must report each month, and what happens if they don't.
  9. Price increasesEscalators, minimum monthly fees and new charges they can add.
  10. Liability and errorsWho pays when a coding or filing mistake costs you money.
