Who bills what
Agree with the office whether you bill payers directly, bill the patient, or bill the office. Put it in writing.
Insurance or self-pay
Cosmetic cases are usually self-pay. Give patients a clear written fee before the day, and collect it the way you've agreed. Insured cases follow the usual anesthesia billing rules.
Documentation still matters
Self-pay or not, keep a full anesthesia record with times. If a cosmetic case becomes medically necessary or a complication happens, you'll need it.
Know the office's setup
Some states and payers have rules for office-based surgery settings. Check the office's accreditation and what your payers require.
