Indiana hasn't opted out of Medicare's physician supervision requirement, so for Medicare purposes CRNAs here work under the supervision of the operating practitioner or an anesthesiologist. Supervision isn't the same as medical direction, though, and that distinction drives the billing. When an anesthesiologist medically directs a case, the CRNA bills with QX and the payment is split. When nobody is medically directing, even with a surgeon supervising, the CRNA's service is billed with QZ.
Groups in Indiana often run a mix of both on the same day, which is exactly where modifier mistakes happen. State law and facility bylaws can add their own rules on top of Medicare's, so the billing has to follow what actually happened in each case.
