Tennessee hasn't opted out of Medicare's physician supervision requirement, so for Medicare purposes CRNAs here work under the supervision of the operating practitioner or an anesthesiologist. Supervision isn't the same as medical direction, though, and that distinction drives the billing. When an anesthesiologist medically directs a case, the CRNA bills with QX and the payment is split. When nobody is medically directing, even with a surgeon supervising, the CRNA's service is billed with QZ.
