When payers try to cap anesthesia time

Every so often a big insurer tries to limit how much anesthesia time it will pay. Here's what's happened and what to watch for.

What has happened

In late 2024, a large Blue Cross Blue Shield insurer announced it would stop paying for anesthesia time beyond a set limit for each procedure in some states. After strong pushback from anesthesia groups, patients and lawmakers, it withdrew the policy within weeks.

Other payers have tried similar things over the years, such as limiting MAC for cataract surgery or endoscopy to certain patients.

How to protect your group

  • Read payer policy updates every month. Your biller should flag anything that affects anesthesia.
  • Make sure your records always show exact times and why the care was needed.
  • Work with your state association. Groups have more weight together.
  • Track payments by payer so you notice if one starts paying less.
